Clinical significance

B19V infection is usually asymptomatic or manifests only in the first phase with flu-like symptoms (e.g., fever, chills, headache, painóle, muscle pain). In the second phase, there may be a rash and itching of the skin, mainly in children (so-called "erythema contagion") and ból of the joints in adults (rheumatoid arthritis). Infection with B19 virus occurs mainly through the droplet route, through blood components and blood products, and through the placenta (fetal infection). The incubation period of the disease is 4-14 days. Within 1-2 weeks after infection, antibodies of the IgM class appear, are present in the blood of the patient for up to about 6 months . Specific IgG antibodies appear in the third week of infection and are detected for years. B19 virus shows tropism to erythrocyteów progenitor cells, to cardiac muscle cells, and to fetal and child hepatocytes. This is due to the affinity of the virus structural proteins for the P (globoside) antigen. This feature of the virus causes, among other things, leukopenia, thrombocytopenia, damage to liver cells, heart muscle, aplastic orifice in the course of hemolytic anemia during the course of infection. In turn, antigen-antibody complexes formed during B10V infection responsible for chronic arthritisów, vasculitisów and glomerulonephritis. Parvovirus B19 is also considered one of the more common causes of pregnancy loss in cases of non-immune fetal edema. Infection in the first twoóch trimesters of pregnancy carries a high risk of intrauterine fetal death. Diagnosis of infection, depending on its phase, is based on serological or molecular tests. The highest viral load is characteristic of the early period of infection, except in immunocompromised patients, in whom the PCR result can be positive for several months and does not indicate acute infection.
